Caregiver and Elder Care Services – Assistance for Aging Parents and Professionals

Erickson Resource Group understands the stress and challenges faced by seniors, caregivers, and their loved ones. We want to make things simpler for individuals and families who struggle to find the time, energy and resources to balance their caregiving responsibilities, work, and their own family obligations.

We know that it is not always easy to access government and community services and to ensure each of those professionals are communicating with one another. Erickson Resource Group is the solution.

Our team of health care professionals is available to work individually with your family member, or as a comprehensive team. We will evaluate your loved one’s individual needs and develop a customized plan or program that fits your family member’s specific needs. We can advocate and assist you in obtaining the government resources that are available and provide the “safety net” for your family until these services are provided. If you choose, we can provide these services as an on-going intervention as well.

All services are available in French and English for those living in Montreal and the surrounding communities. All team members are available to meet at your place of residence.

After the initial phone contact, one of our team members will visit your friend or family member in their own environment to assess their individual needs. We will ensure that each client’s needs are reviewed by the appropriate team member and that the most appropriate professional is the lead Care Manager for that case. Some services, such as social work, nursing, dietitian, and occupational therapy, may be covered by private insurance.

MandatePsychosocial assessments for mandate homologation and curatorship applications
These legal documents are completed by a social worker within 10 days of meeting with the client. An expedited service is available (5 days) if needed. We will work collaboratively with your notary or attorney and can provide referrals if necessary.

PlacementsRelocation and home care guidance
A social worker is available to guide families through the relocation process when moving a loved one into an assisted or long term care facility for seniors. We can also assist families in negotiating the public health care system and accessing public or private services for home care and other long term care services.

LiasonLiaison for out of town family
For families and friends who do not live locally, a social worker is available to assess, organize and supervise your aging parent or loved one’s care whether they are at home or in a facility. We will provide weekly, bi-monthly or monthly visits to your loved one and communicate with you after each contact to ensure you have up to date information about your family member’s functioning. We can assist you in advocating and obtaining care within the public or private health care system.

LiasonOccupational Therapy
An occupational therapist is available to assist clients with the following:

  • Physical capacities (includes strength, coordination, balance, self-care, mobility and/or other physical abilities);
  • Safety evaluation (mobility, ambulation, domestic tasks, personal care, etc.)
  • Cognitive capacities (including memory, organization skills, coping strategies);
  • Driving assessments (safety evaluation) – revocation of driver’s license if necessary
  • Advocate and supports clients and their caregivers in obtaining the necessary support for special equipment, transportation and adapted housing from government agencies.

LiasonRecreational Therapy
A recreational therapist is available to assist patients and their caregivers with improving the mental and cognitive stimulation of an individual suffering with a cognitive disorder, such as Alzheimer’s disease and dementia. Our therapist can develop a personalized activity plan for your parent and/or loved one that includes personalized activities that meet their unique needs and interests. Our therapist will instruct and teach the family or the paid caregivers on how to integrate these activities into the individuals’ daily routine.

Home

A dietitian knows that elderly individuals face unique nutritional challenges and they are at risk for nutritional deficits that can affect their quality of life. Nutritional needs can be compromised from many issues such as arthritis, osteoporosis, diabetes and other physical illnesses; mental health; eating problems like tooth loss, mouth pain, chewing and swallowing; digestion and absorption issues; multiple medications; and weight loss/gain. In addition, seniors’ appetites can diminish overtime, yet their body requires increased nutrients. Our dietitian can assess your loved one’s unique nutritional and dietary needs, evaluate their current regime, and create a meal and shopping plan to assist your loved one in improving their nutritional needs. Our dietitian can facilitate a “shopping trip” to educate the senior or caregiver on how to shop for the appropriate foods.

NursingA Registered Nurse is available to visit your parent or loved one in his or her home for assessments and interventions related to home care needs, loss of autonomy, the teaching of patients and caregivers about medical diagnoses and symptoms, medication issues and direct service (injections). In addition, she can assist patients accessing their appointments with multiple sub-specialists, such as private medical care, general practitioners, dental care and social services.
